Job summary

VNA Home Health & Hospice in Rhode Island is seeking an HH clinician to manage a caseload from admission through transition into the community, coordinating interdisciplinary support and referrals as appropriate.

Under supervision of the Team/Program Manager, you will monitor and adjust treatment to achieve positive outcomes and identify opportunities for process improvement, in line with organization policies and federal/state accreditation standards.

Qualifications

• Requires RI licensure eligibility and RN license.
• Bachelor’s degree in a relevant field strongly preferred.
• CPR certification required.

Responsibilities

• Manage a caseload from admission to community transition.
• Coordinate interdisciplinary care and referrals as appropriate.
• Monitor and adjust treatment to achieve positive outcomes.
