Job summary

Care New England Health System is seeking a field clinician supervised by the Team/Program Manager to manage a caseload of patients from admission to community transition. The role involves coordinating interdisciplinary care, arranging referrals when appropriate, and ensuring care aligns with organization policies and accreditation standards.

You will monitor and adjust treatment to achieve positive outcomes and identify opportunities for process improvement in collaboration with the care team.

Qualifications

• RI licensure required or equivalent education.
• Bachelor’s degree (min) in a relevant discipline strongly preferred.

Responsibilities

• Manage a caseload of patients from admission through transition into the community.
• Coordinate interdisciplinary care and referrals as appropriate.
