The Care Team Associate II supports providers with clinical services and administrative tasks to help sustain population health guided best in class care.

Qualifications

• High School diploma or GED required.
• Associate degree preferred.
• Certificate of completion from an accredited Medical Assistant Program required or eligibility to sit for the national certification exam and pass within 18 months of hire.

Responsibilities

• Greet patients and manage registration and check-out.
• Answer phones with professional customer service and schedule appointments.
• Process patient demographics, insurance verification, and data entry.
• Assist care teams with patient care under RN/LPN supervision.
• Maintain accurate documentation and EMR data quality.
