The Care Team Associate I supports the team with non-clinical services and administrative tasks focused on population health, under supervision of the Practice

Qualifications

• High School diploma or GED required.
• Associate degree preferred for advancement.
• Experience in customer service or care coordination is valuable.

Responsibilities

• Provide general nursing care including medications and treatments per orders.
• Greet patients, manage wait times, and assist with registrations and check-out.
• Answer calls, schedule appointments, verify insurance, and resolve patient issues.
• Register patient demographics and financial information accurately in electronic systems.
• Coordinate referrals to specialists and arrange tests and follow-ups.
• Maintain data integrity and ensure timely communication with patients and staff.
