St Anne's Community Services is seeking a full-time Care Coordinator to oversee the care planning system at Redwood Glades. You will manage day-to-day care calls, ensure continuity of care for clients, and provide operational oversight of scheduling and service delivery.

The role typically runs Monday to Friday, with flexibility to cover additional hours and to adjust to changing client needs. You will plan rosters at least four weeks in advance and allocate care workers according to

• Good knowledge of IT systems.
• Experience using rota systems is desirable.

Responsabilidades

• Plan and allocate care workers to service user calls and manage workload.
• Oversee the care planning system and ensure calls are assigned in line with policy.
• Take new service user referrals, liaise with brokerage for package increases, and ensure smooth transitions of care.
