Job summary

Grace Community Care and Homes Inc. is seeking a part-time care coordination professional to match clients with qualified staff and monitor coverage across assignments.

You will reconcile schedules with authorizations and coordinate visit documentation, ensuring confidentiality and adherence to scope and policies. Collaborating with intake, credentialing, billing and quality teams, you will participate in orientation, competency reviews and ongoing quality improvement.

• Experience in home-care scheduling and coordinating with authorization workflows.

Responsibilities

• Match clients and qualified staff, monitor coverage, manage call-outs, reconcile schedules with authorizations and coordinate visit documentation.
• Work collaboratively with department leadership, intake, credentialing, billing and quality teams. Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow role-specific policies and professional scope. Participate in orientation, competency review and quality improvement.
