Qualificações

• Bachelor’s degree in health-related field preferred; High School diploma required.
• 1 year of customer service experience or 2 years in a public/private social service or healthcare setting.
• Basic familiarity with medical terminology.
• Intermediate proficiency with Microsoft Office (Word, Excel, Outlook).

Responsabilidades

• Engage patients in collaborative ongoing relationship to facilitate care.
• Promote ACCESS programs and services; provide health information for self-management.
• Conduct home visits and case management activities as needed.
• Coordinate care with an interdisciplinary team and track outcomes.
