Job summary

Thresholds-Inc in Chicago is seeking a Care Coordinator for CCBHC services. You will coordinate referrals, link members to external providers, and support recovery goals through outreach and case management.

The role emphasizes data quality, reporting, and collaboration with the Outpatient team to ensure timely access to services for members. A valid driver’s license and strong community engagement experience are required.

Qualifications

• Must have a high school diploma and at least two years of mental health and/or community engagement experience.
• Mental Health Professional designation (Rule 132) strongly preferred.
• Qualified Mental Health Professional with a Master’s degree in social work, counseling, or closely related field is a plus.
• Experience working with persons diagnosed with mental illness preferred.
• Ability to document services in Electronic Health Records in a timely manner.

Responsibilities

• Monitors and facilitates access, engagement and linkage to healthcare services for Thresholds CCBHC members.
• Provides case management by phone and in person to refer and link members to external providers and community resources.
• Conducts community outreach to assist linkage to mental health, physical health, substance use services, and other resources.
• Follows up with collateral providers to improve continuity of care.
• Addresses barriers to care when members miss appointments.
• Documents member care and communication in the electronic health record per requirements.
• Identifies and links members to Thresholds IPS program and assists with benefits applications.
• Completes National Outcome Measures (NOMS) timely.
