Job summary

The Nurse Case Manager at Chicago Family Health Center will provide direct patient care through implementation of individual care plans developed with the patient’s primary care provider. You will work with the provider for continuity of care and promote self-care management, using face-to-face contact, telephone follow-up and education under care guidelines.

Schedule is Monday to Friday, 8:30am-5:15pm. We value collaborative teamwork, community health focus, and compassionate, confidential care

Qualifications

• Associate degree in Nursing required; Bachelor’s degree to facilitate complex disease care management and advanced patient needs.
• Licensed Practical Nurse (LPN) or Registered Nurse (R.N.) with a valid license in Illinois
• Current CPR Certification
• Two to three years’ experience as a qualified Case Manager preferred
• Significant experience in community-based environments and with complex care packages

Responsibilities

• Provide ongoing case and care management for an assigned population (Population Health Management).
• Perform Comprehensive Risk Assessments and develop individualized Patient Care Plans.
• Coordinate patient access to care and oversee Care Coordinators and referrals.
• Address transitions of care when patients obtain hospital-based care.
• Collaborate in Patient Care Teams and support population health initiatives.
