Access Community Health Network in Illinois seeks a dedicated Care Coordinator (RN, Social Worker or Counselor) to help patients navigate transitions, coordinate care, and promote health education. The role leads intake, develops care plans, tracks patients by disease and risk, and supports smooth outpatient-to-inpatient transitions, with emphasis on patient empowerment and self-management.

• Bachelor’s degree required; Master’s degree preferred.
• Illinois license LCPC, LPC, LCSW or LSW required for social work path.
• Minimum two years in a clinical setting.

Responsabilidades

• Serve as team lead for intake and care plan development.
• Promote health programs and manage a patient panel.
• Lead patient-centered care planning and evaluation.
• Conduct face-to-face visits per policy and workflows.
• Support transitions from outpatient to inpatient and ensure follow-up.
