Job summary

Ramey-Estep Homes, Inc. seeks a Care Navigator to serve as a central contact for behavioral health, primary care, and community supports. The role blends in-person engagement with telehealth and administrative duties to ensure timely, person-centered care.

Collaborating with clinicians, case managers, and partners, the navigator helps reduce barriers, coordinate services, and promote recovery, wellness, and long-term stability in line with CCBHC standards.

Qualifications

• High school diploma or equivalent required.
• Associate's or Bachelor's degree in healthcare, social services, or related field preferred.
• Knowledge of community resources and health systems preferred.
• Experience in behavioral health, case management, or navigation is a plus.

Responsibilities

• Welcome and support clients in-person, via phone, or telehealth.
• Conduct initial screenings to determine eligibility and service needs.
• Complete intake forms, consent, and release documents (HIPAA/42 CFR Part 2 compliant).
• Educate clients on available services, rights, grievance procedures, and community resources.
• Connect clients to behavioral health, primary care, and community support services.
• Track and follow up on referrals to ensure successful connection.
• Help clients overcome barriers such as transportation, housing, employment, insurance, or documentation.
• Collaborate with clinical and case management teams on individualized care plans.
• Maintain consistent contact with clients through calls, virtual check-ins, and in-person visits.
• Support clients in setting and achieving personal goals.
• Monitor client progress, identify unmet needs, and escalation risks to the care team.
• Document client interactions, progress notes, referrals, and care plan updates in the EHR.
• Track outcomes using agency-approved tools (PHQ-9, GAD-7, CANS, etc.).
• Provide in-person support for high-need clients, intakes, and community events.
• Conduct telehealth check-ins and administrative follow-up remotely.
• Adapt flexibly between fieldwork and virtual tasks to meet client and program needs.
