Ramey-Estep Homes, Inc. is seeking a Care Navigator to be the central contact for clients navigating behavioral health, primary care, and community services.

This role blends in-person engagement with remote telehealth and administrative support to ensure timely, person-centered care and recovery. You will collaborate with clinicians, case managers, and partners to reduce barriers, coordinate services, and promote stability, while adhering to CCBHC standards.

Competenze

• High School Diploma or equivalent required.
• Associate’s or Bachelor’s degree in healthcare, social services, or related field preferred.

Mansioni

• Welcome clients in-person, via phone, or telehealth.
• Conduct initial screenings to determine eligibility and service needs.
• Coordinate services and track referrals to ensure connection.
• Document interactions and care plans in the EHR.
• Support client safety, transitions, and discharge planning.
