Qualifications

Responsibilities

• Assist patients within the network who are high or rising risk and eligible for additional healthcare support.
• Act as patient advocate and navigator; conduct comprehensive screenings and engage patients.
• Connect patients with network providers, payor resources, and relevant services.
• Coordinate care plans and communicate across interdisciplinary teams and community resources.
• Support continuity of care across inpatient/outpatient settings and various care facilities.
