Job summary

Ramey-Estep Homes, Inc. is seeking a Care Navigator to serve as a central point of contact for individuals navigating behavioral health, primary care, and community services.

This role blends in-person engagement with telehealth and administrative support to help clients access timely, person-centered care. You will collaborate with clinicians, case managers, and community partners to coordinate services, reduce barriers, track referrals, and support recovery, wellness, and long-term stability

Qualifications

• Must be at least 21 years of age.
• Strong leadership and organizational skills.
• Valid Driver's License and insurability.
• Excellent communication and conflict-resolution skills.
• Proficient with telehealth platforms and MS Office applications (Word, Excel, PowerPoint).
• Ability to engage clients from diverse backgrounds.
• English reading/writing/listening proficiency.
• Pre-employment drug screen and background check.
• TB test or chest x-ray or equivalent documentation.

Responsibilities

• Welcome and support clients in-person, via phone, or telehealth.
• Conduct initial screenings to determine eligibility and service needs.
• Complete intake forms, consent, and release documents (HIPAA/42 CFR Part 2 compliant).
• Educate clients on services, rights, grievance procedures, and resources.
• Coordinate behavioral health, primary care, and community services.
• Track and follow up on referrals for successful connections.
• Assist with transportation, housing, employment, and insurance barriers.
• Collaborate with clinical teams on individualized care plans.
• Maintain contact via calls, virtual check-ins, and visits.
• Support client goal setting and progress monitoring.
