Job summary

The Care Navigator at Ramey-Estep Homes, Inc. serves as the central contact for individuals seeking behavioral health, primary care, and community support services.

The role blends in-person engagement with remote telehealth and administrative tasks to ensure timely, person-centered care aligned with CCBHC standards. Collaborating with clinicians, case managers, and partners, the navigator helps remove barriers, coordinates services, and supports recovery, wellness, and long-term stability

Qualifications

• High School Diploma or equivalent required.
• Associate or Bachelor’s Degree in healthcare or related field preferred.

Responsibilities

• Welcome and support clients in-person, via phone, or telehealth.
• Conduct initial screenings to determine eligibility and service needs.
• Educate clients on available services, rights, grievance procedures, and community resources.
• Connect clients to behavioral health, primary care, and community support services.
• Track and follow up on referrals to ensure successful connection.
• Collaborate with clinical and case management teams on individualized care plans.
• Document client interactions and progress in the EHR.
