Job summary

Ramey-Estep / Re-group is seeking a Care Navigator to serve as a central contact for individuals accessing behavioral health, primary care, and community services. The role blends in-person engagement with remote telehealth and administrative support to ensure timely, person-centered care in alignment with CCBHC standards.

You will collaborate with clinicians, case managers, and community partners, helping clients overcome barriers and maintain continuity of care.

Qualifications

• High School Diploma or equivalent required.
• Associate's or Bachelor's in healthcare, social services, or related field preferred.
• Knowledge of community resources and health systems beneficial.
• Behavioral health navigation experience preferred.

Responsibilities

• Welcome and support clients in-person, via phone, or telehealth.
• Conduct initial screenings to determine eligibility and service needs.
• Complete intake forms, consent, and release documents (HIPAA/42 CFR Part 2 compliant).
• Educate clients on services, rights, grievance procedures, and community resources.
• Connect clients to behavioral health, primary care, and community support services.
• Track and follow up on referrals to ensure successful connections.
• Collaborate with clinical and case management teams on care plans.
• Maintain regular contact with clients through calls, virtual check-ins, and in-person visits.
