Community Reach Center is seeking a compassionate Crisis Navigator (CN) to provide clinical and support services to individuals in mental health crises. The CN will facilitate care navigation, linking consumers to ongoing treatment and resources, while coordinating with emergency services.

Qualifications

• Requires bachelor's degree in psychology, Social Work, or directly related field.
• 6 months of experience with adults who have mental illness preferred.
• Ability to work independently in a community-based setting.
• Valid Colorado Driver’s license with a clean driving record.

Responsibilities

• Provide prevention/early intervention and care navigation to consumers.
• Support and coordinate mental health care with the consumer's clinical team.
• Document interventions and track patient follow-up using EHR.
• Provide therapeutic crisis intervention and emergency services as needed.
• Assist eligible consumers in gaining access to needed services.
