Job summary

Rimrock Foundation in Billings, MT seeks an Integrated Services Targeted Case Manager to coordinate SUD, medical, and behavioral health care for patients. You will act as a system navigator, support families, and promote adherence to regulations while representing the organization.

Under supervision, you will collaborate with a multidisciplinary team, assist with paperwork, coordinate appointments, advocate for needed services, and contribute to health home operations and quality improvement.

Qualifications

• Bachelor's Degree in Human Service or Social Work required.
• One year of experience in Mental Health or SUD field preferred.

Responsibilities

• Assist new patients with paperwork and releases of information.
• Act as the system navigator and point of contact for patients and families.
• Assist patients and families in problem solving barriers to healthcare.
• Assist patients during medical appointments and provide education on care needs.
• Schedule appointments with external medical specialists and explain purposes clearly.
• Collaborate with multidisciplinary team for health home services.
• Coordinate with Rimrock team and community services to address needs.
• Teach patients to manage chronic diseases with SMART goals.
• Advocate for necessary supplies/services for patients.
