Job summary

Rimrock is seeking an Integrated Services Targeted Case Manager to coordinate comprehensive care for patients with SUD, medical, and mental health needs. This role emphasizes a holistic, whole-person approach and adherence to state statutes, CARF standards, and HIPAA regulations.

The TCM will act as a navigator and contact for patients and families, coordinating with the medical team and community services to address both disease management and prevention.

Qualifications

• Excellent communication and collaboration with a multidisciplinary team.
• Ability to educate patients and families about care plans and medications.
• Experience with HIPAA and privacy regulations and maintaining confidentiality.

Responsibilities

• Assist new patients in paperwork and releases of information needed to transfer medical care.
• Act as system navigator and point of contact for patients and families.
• Assist patients and families with barriers to care including transportation and prescription assistance.
• Educate patients during medical visits to improve understanding of diseases and treatment plans.
• Schedule external appointments and communicate purpose in clear terms.
• Participate in a multi-disciplinary team to manage health home operations.
• Coordinate with Rimrock and community services to address health needs and promotion.
• Teach patients to self-manage chronic diseases using SMART goals.
• Assist in budgeting related to medical needs and refer to Integrated Services Lead as needed.
• Use step-by-step teaching methods and culturally-sensitive approaches.
• Attend trainings and provide feedback on educational content.
