Qualifications

• Master’s Degree in a human services related field preferred.
• Bachelor’s degree in a human services related field with 1 year relevant experience preferred.
• High School Diploma or GED with at least 3 years of experience working with the target population.

Responsibilities

• Engage individuals experiencing acute behavioral health or substance-related crises using trauma-informed, recovery-oriented approaches.
• Conduct care management activities including screening, needs identification, service planning, and coordination across systems of care.
• Support crisis stabilization by facilitating referrals to outpatient care, inpatient hospitalization, residential treatment, detoxification, or community-based supports.
