Job summary

Qualifications

• Bachelor's Degree or equivalent combination of education and experience in the Human Services field.
• Must complete Case Management Training and provide certificate within 6 months of hire.
• Ability to work with persons with serious emotional disturbance and/or severe and persistent mental illness; possess competency in conducting highly-organized and detail-oriented work.

Responsibilities

• Builds referral networks and coordinates with internal and external providers to improve outcomes and quality of care.
• Maintains trauma-informed, culturally competent interactions with clients and families.
• Documents client information timely in required systems and communicates with care teams.
• Opens new client charts and facilitates intake and initial assessments; explains policies and financial considerations.

Education

Bachelor's Degree or equivalent combination of education and experience in the Human Services field
