Job summary

Master•Care, Inc. is a California MSO focused on Enhanced Care Management, Housing Navigation, and Nursing Facility Transition.

We seek a Care Navigator to provide non-clinical care management under the Master•Care Plan, coordinating with medical and community providers and supporting whole-person well-being. The role requires in-person and remote service within the Stanislaus County region, with a salary of $25-$28 per hour and a benefits package.

Qualifications

• Bachelor’s degree or equivalent experience in healthcare- or senior services-related field.
• 3+ years in marketing and/or social services within healthcare or senior services.
• Knowledge of clinical and non-clinical services for elderly populations.

Responsibilities

• Serve as primary contact with local medical and nonmedical providers.
• Develop referral relationships to support company objectives.
• Conduct Comprehensive Assessments for ECM and CS patients.
• Develop and execute Master Care Plans.
• Coordinate in-home or facility assessments as needed.
