Daymark Recovery Services is seeking a Care Management Extender to support the Albemarle center. Under supervision, you will assist with care management tasks, referrals, documentation and follow-up to help members access needed services and maintain health goals.

The role emphasizes crisis intervention, ensuring a coordinated plan of care and collaboration among providers. A high school diploma is required with eligible candidates gaining experience in behavioral health services.

Qualifications

• Minimum of a high school diploma or equivalent.
• Meet one of the following criteria: Certified Peer Support Specialists; Community health workers (CHW); individuals who served as Community Navigators; parents/guardians of an individual with an I/DD or TBI; a person with lived experience with an I/DD or a TBI or behavioral health condition; or 2 years of paid care management experience with at least 1-year paid experience with the population served.

Responsabilités

• Provides care management extender duties, referring and linking to needed services, monitoring/follow up with client and referrals, provide education for health promotion.
• Participates in interdisciplinary treatment planning, consultation activities and ensures all involved parties are aware of the plan of care.
• Provides crisis intervention to all participants of TCM and involves crisis services when needed.
• All other duties as assigned by supervisor.
