Job summary

ProMedica Toledo Hospital in Toledo, OH is seeking a Social Worker/Discharge Planning Coordinator for per diem daytime hours to support high-risk patients and caregivers. You will assess psychosocial risk, coordinate discharge plans, and advocate for patients during hospitalization, connecting them with community resources.

You must have a BSW or higher degree (MSW preferred), strong knowledge of regulations, excellent communication, and experience with electronic medical records and care

Qualifications

• BSW or higher degree required; MSW preferred.
• Knowledge of regulations, requirements, and community resources.
• Excellent written and verbal communication; ability to work in a diverse environment.
• Experience with electronic medical records and care transition processes.
• Previous case management or discharge planning experience is a plus.

Responsibilities

• Assess psychosocial risk factors for patients and caregivers.
• Coordinate and facilitate development of discharge plans for high-risk patients.
• Advocate for patients and ensure continuity of care during hospitalization.
• Connect patients to community resources for emotional, social, and financial support.
• Address psychosocial concerns such as mental health, substance use, and guardianship.
