Job summary

Mercy seeks a Care Manager to join an interdisciplinary team to assess, plan, advocate, and coordinate care from admission to discharge, ensuring a safe transition post hospitalization and maintaining patient experience, safety, and quality of care. The Care Management model utilizes metrics and best practices to guide transitions.

Requires a Master's in Social Work or MSW candidacy, 0–2 years in an acute care setting, LMSW/LCSW licensure, BLS CPR, and Certification in Case Management.

Qualifications

• Master's in Social Work or completion of MSW requirements (for hires after 2011).
• 0–2 years' experience in an acute care hospital setting.
• Current LMSW or LCSW license in the state of employment.

Responsibilities

• Assess, plan, advocate and coordinate care from admission to discharge to ensure safe transitions.
• Improve patient experience, safety and quality of care through care coordination.
• Collaborate with interdisciplinary team within Mercy’s care management model.
