Job summary

Molina Healthcare, Inc. is seeking an experienced Care Manager RN to support care management activities and coordinate member care across settings. You will work with a multidisciplinary team to ensure progress toward outcomes, utilizing motivational interviewing and clinical guidelines.

The role involves 25–40% local travel, conducting assessments, developing care plans, and coordinating with caregivers, physicians, and other health professionals to optimize quality and cost-effective care.

Qualifications

• At least 2 years experience in health care, care management or related settings.
• RN license active and unrestricted in state of practice.
• Valid driver’s license and reliable transportation for travel requirements.
• Understanding of EMR systems and HIPAA.
• Ability to work independently with minimal supervision.

Responsibilities

• Completes comprehensive member assessments within regulated timelines.
• Develops and implements care coordination plans with care team and network.
• Performs telephonic, in-person, or home visits as required.
• Monitors care plans, documents interventions, and suggests changes.
• Maintains caseload with regular outreach and care coordination.
• Promotes integration of behavioral health, LTSS, and home resources.
