Job summary

Molina Healthcare, Inc. is seeking a Care Manager RN to support care management and coordinate services across the continuum. You will help ensure member progress toward outcomes and contribute to a strategy for quality, cost-effective care.

Responsibilities include comprehensive assessments, care coordination planning with the member network, and ongoing monitoring of care plans. Travel up to 40% may be required depending on state contracts.

Qualifications

• 2+ years of experience in health care, preferably in care management or equivalent.
• Active RN license in state of practice.
• Valid driver's license and reliable transportation for travel requirements.
• Knowledge of EMR systems and HIPAA regulations.
• CCM certification is preferred.

Responsibilities

• Complete member assessments within required timelines and determine care management eligibility.
• Develop and implement care coordination plans with the care team and member network.
• Conduct telephonic, in-person, or home visits as needed.
• Monitor and adjust care plans based on outcomes and interventions.
• Maintain a ongoing member caseload and perform regular outreach.
• Promote integration of behavioral health, LTSS, and community resources.
• Facilitate ICT meetings and collaboration within the care team.
• Educate and motivate members using motivational interviewing techniques.
• Identify barriers to care and provide navigation support.
• Provide consultation and resources to peers when needed.
• Care manager RNs may handle complex cases and medication reconciliation.
• Travel 25-40% based on state/contractual requirements.
