Job summary

Community Care of North Carolina Inc is seeking a Care Manager (RN or Social Worker track) to assess, plan, implement, coordinate, monitor, and evaluate care options for enrolled populations. You will collaborate with PCPs, families, and the care team to deliver a full continuum of health care services, considering holistic needs.

The role may be performed remotely within regions and requires travel across the state to serve Regions 2, 4, and 6, with preference for NC counties such as Davie,

Qualifications

• Active, unrestricted RN license to practice in North Carolina.
• Master’s degree in social work (MSW) required for social worker track.
• CCM certification preferred; will obtain within 1 year of eligibility per CCM requirements.
• Access to Hospital/Data or EMR systems required.

Responsibilities

• Provide comprehensive care management based on case management standards of practice for enrolled populations.
• Collaborate with the Primary Care Provider, member, guardian, caregivers, family members, and care team to coordinate services across the continuum of care.
• Develop, review, implement, and evaluate the member care plan with SMART goals in partnership with the member and care team.
• Maintain HIPAA-compliant documentation in the care management system.
• Travel within the region and/or state; may work remotely within regions to cover needs.
