Job summary

Community Care of North Carolina Inc. is seeking a Care Manager to assess, plan, coordinate, and monitor care for enrolled populations in Cary, NC. The role collaborates with primary care providers, caregivers, and a multi-disciplinary team to deliver holistic, cost-effective health outcomes.

The position may be performed remotely within regions, with travel across the state as needed. Licensure as RN in NC or LCSW is required, with CCM certification preferred and obtainable within a year.

Qualifications

• Excellent communication skills – oral and written
• CCM certification preferred; will obtain within 1 year among CCM requirements
• Active North Carolina RN license or NC LCSW license
• BSN preferred for RN track; MSW for Social Worker track

Responsibilities

• Provide effective Care Management services per standards of practice.
• Complete member assessments covering medical, psychosocial, behavioral, and cultural needs.
• Develop and evaluate member care plans with member, family, and care team.
• Facilitate referrals to community-based services and coordinate with providers.
• Maintain documentation in the Care Management system and meet productivity expectations.
