Job summary

Community Care of North Carolina (CCNC) is seeking a Care Manager 1 - Non-Clinical to support Medicaid enrolled members across NC. This field-based role may involve remote work when not in the field, with occasional in-person training and travel within the state.

The position emphasizes holistic assessment, care planning, and collaboration with a multi-disciplinary team to ensure integrated, high-quality care and cost-effective outcomes.

Qualifications

• Bachelor’s degree in a health-related field or licensure as an RN.
• 2 years experience with individuals served by child welfare preferred.
• Must reside in NC or within 40 miles of the NC border.
• CCM certification preferred.
• Valid driver’s license with current auto liability insurance.

Responsibilities

• Provide statewide care management to support Medicaid enrolled members.
• Assess, plan, implement, coordinate, monitor, and evaluate care options.
• Collaborate with the Primary Care Provider, member, guardian, caregivers, and family.
