Job summary

Candidates must have an active NC RN license and at least 2 years of clinical experience. Remote work, schedule Mon-Fri, with limited travel where applicable, are key highlights of this role.

Qualifications

• Active, unrestricted RN license in North Carolina.
• Must reside in North Carolina.
• Minimum 2 years of RN clinical experience.
• Associate Degree in Nursing.
• Experience in multiple clinical settings (ICU, NICU, ED, Med/Surg, Telemetry, OR, Home Health, Hospice, LTC, Infusion, Orthopedics, and more).
• Strong communication and member engagement skills.

Responsibilities

• Conduct telephonic assessments to identify members' medical, behavioral, and social needs.
• Develop and coordinate personalized care plans to support overall wellness.
• Collaborate with healthcare providers and multidisciplinary teams to remove barriers to care.
• Utilize clinical judgment to address complex health conditions and improve outcomes.
• Support members through transitions of care and connect them with appropriate resources.
• Maintain compliance with regulatory and company case management standards.
