Job summary

ResCare Community Living, a member of Sevita, seeks a dedicated Care Manager support professional in North Carolina to deliver integrated care management across physical health, behavioral health, I/DD, TBI, LTSS and pharmacy needs. Partner with the Care Manager to execute plans and coordinate services for members in the community.

Qualifications

• Must be at least 18 years old and have a high school diploma or GED.
• Experience with Tailored Care Management preferred.
• Knowledge of North Carolina Medicaid delivery system or caregiver experience preferred.

Responsibilities

• Assist Care Manager in delivering integrated care management across health domains.
• Engage members and families professionally via telephonic, virtual, and in-home visits.
• Document billable and care events and maintain up-to-date electronic records.
• Coordinate services/appointments and participate in planning meetings.
• Identify barriers and support Medicaid Tailored Care Management needs.
