Job summary

Independent Living Systems, LLC seeks a Care Manager to coordinate comprehensive care for eligible individuals in Florida. You will assess needs, develop person-centered care plans, and ensure services promote health, independence, and quality of life.

You will collaborate with healthcare providers, families, and community resources, monitor progress, and adjust plans to meet changing needs while aligning with guidelines and program requirements.

Qualifications

• Bachelor’s degree in social work, sociology, psychology, gerontology, or related field.
• RN or LPN license to practice in Florida depending on role.
• Two years of relevant experience required.
• EHR proficiency and basic computer applications.

Responsibilities

• Serve as primary point of contact for members and authorized representatives.
• Assess member needs, identify care gaps, and develop person-centered care plans.
• Monitor progress and adjust care plans to address changing needs.
• Coordinate services across the continuum and communicate with providers and resources.
• Educate and support members on available resources and self-advocacy.
• Maintain documentation and ensure compliance with policies and standards.
• Perform other duties as assigned.
