Job summary

Independent Living Systems in Marathon, FL is seeking a Care Manager to coordinate comprehensive long‑term care for eligible individuals. You will assess needs, develop person‑centered care plans, coordinate services, and advocate for members across the care continuum.

This role requires a Florida‑licensed RN or LPN preferred, a relevant social services degree, experience with EHR systems, and strong collaboration with providers, families, and community resources to support optimal outcomes.

Qualifications

• Bachelor’s degree in social work, sociology, psychology, gerontology, or related social services field.
• Bachelor's degree in a field other than social science.
• Registered Nurse (RN) licensed to practice in Florida.
• Licensed Practical Nurse (LPN) with 4+ years licensed in Florida.
• Proficiency in electronic health records (EHR) and basic computer applications.
• Relevant professional human service experience may substitute for educational requirements on a year-for-year basis.

Responsibilities

• Assess member needs, identify care gaps, and assist with development of person-centered care plans.
• Monitor progress and adjust care plans to address changing needs.
• Coordinate services across continuum and facilitate communication with providers and community resources.
• Serve as primary contact for members and authorized representatives.
• Provide education and support on available resources and self-advocacy.
• Maintain accurate documentation and ensure compliance with policies and quality standards.

Education

Bachelor’s degree in social work or related field

Bachelor's degree in a field other than social science

Master’s degree in social work or public health (preferred)

CCM Certification (preferred)

RN licensed in Florida

LPN licensed in Florida (4+ years)

Proficiency in EHR and basic computer applications
