Job summary

Independent Living Systems (ILS) is seeking a Care Manager to join our St. Petersburg, FL team. This role focuses on coordinating medically necessary long-term care services for eligible individuals, ensuring person-centered planning and seamless access to care.

You will assess needs, develop care and discharge plans, coordinate across providers, and monitor progress with families and teams to promote health, independence, and quality of life.

Qualifications

• Minimum 2 years of relevant experience.
• Bachelor’s degree in social work, sociology, psychology, gerontology, or related social services field.
• RN licensed in Florida or LPN with 4+ years licensed experience in FL; relevant experience may substitute education.

Responsibilities

• Assess member needs and develop person-centered care plans.
• Coordinate services across continuum of care and facilitate transitions.
• Monitor progress and adjust care plans to changing needs.
• Collaborate with healthcare providers, social workers, families, and community resources.
• Maintain accurate documentation and ensure compliance with policies and standards.
