Job summary

Independent Living Systems (ILS) is seeking a Care Manager to coordinate comprehensive care for eligible individuals requiring medically necessary long-term care services. You will assess needs, develop person-centered care plans, and monitor progress to ensure quality outcomes across the care journey.

We collaborate with healthcare providers, families, and community resources to advocate for members, align care with clinical guidelines, and support independence and well-being in Florida

Qualifications

• Bachelor’s degree in social work, sociology, psychology, gerontology, or related social services field.
• Bachelor's degree in a field other than social science.
• Registered Nurse (RN) licensed to practice in Florida or LPN with at least 4 years licensed in Florida.

Responsibilities

• Assess member needs, identify care gaps, and assist in developing person-centered care plans.
• Monitor progress and adjust care plans to address changing needs and ensure continuity of care.
• Coordinate services across the continuum and facilitate communication with providers and community resources.
• Provide education and support on available resources and self-advocacy.
• Maintain accurate documentation and ensure compliance with policies and regulations.
• Serve as primary point of contact for members and authorized representatives.
