Job summary

Independent Living Systems (ILS) is seeking a Care Manager to coordinate comprehensive care for eligible individuals in need of long-term services and supports. You will work with multidisciplinary teams to assess needs, develop person-centered care and discharge plans, and monitor progress to ensure optimal outcomes across the member’s care journey.

The ideal candidate will possess relevant experience with long-term care, standbys for Florida licensure, and strong collaboration with families

Qualifications

• Two years of relevant experience required.
• Bachelor’s degree in social work, sociology, psychology, gerontology or related field.
• RN licensed in Florida or LPN with four years in Florida.
• Professional human services experience may substitute education on a year-for-year basis.
• Proficiency in EHR and basic computer applications.

Responsibilities

• Assess member needs, identify care gaps, and develop person-centered care plans.
• Coordinate services across the continuum and facilitate transitions between care settings.
• Collaborate with healthcare providers, social workers, families, and community resources.
• Monitor progress and adjust care plans to address changing needs and outcomes.
• Maintain documentation and ensure compliance with policies and standards.
• Advocate for members and liaise with care delivery systems.
