Qualifications

• Must have at least two years of relevant experience.
• Experience coordinating care and working with multidisciplinary teams preferred.
• Familiarity with electronic health records and basic software applications.

Responsibilities

• Serve as primary contact for members and authorized representatives.
• Assess needs and develop person-centered care plans.
