Job summary

Independent Living Systems is seeking a Care Manager in Lake Placid, FL to coordinate medically necessary long-term care services for eligible individuals. You will assess needs, develop person-centered care plans, coordinate services, and oversee transitions across care settings.

Ideal candidates have a related bachelor’s degree and at least two years of experience; RN or LPN licensure in Florida is a plus. Proficiency with EHR systems and strong advocacy skills are valued.

Qualifications

• Bachelor’s degree in social work, sociology, psychology, gerontology, or related social services field.
• Two years of relevant experience or substitution as allowed.
• Proficiency in EHR and basic computer applications.

Responsibilities

• Assess member needs and develop person-centered care plans.
• Coordinate services across continuum of care and community resources.
• Monitor progress and adjust care plans as needed to ensure continuity of care.
• Serve as primary contact for members and authorized representatives.
• Maintain documentation and ensure regulatory compliance.
