Job summary

Independent Living Systems (ILS) is seeking a Care Manager to coordinate medically necessary long-term care services and supports for eligible individuals in Florida. You will assess needs, develop person-centered plans, and collaborate with healthcare providers, families, and community resources to promote health and independence.

rove patient advocacy, ensure alignment with clinical guidelines, and monitor progress to adjust plans as needed for optimal outcomes throughout the care journey.

Qualifications

• Bachelor’s degree in social work, sociology, psychology, gerontology or related field.
• RN or LPN license valid to practice in Florida.
• Minimum of 2 years of relevant care coordination experience.
• Proficiency with electronic health records (EHR) and basic computer apps.

Responsibilities

• Assess member needs and assist in developing person-centered care plans.
• Coordinate services across the continuum and with providers and families.
• Monitor progress and adjust care plans to ensure continuity of care.
• Serve as primary contact for members and authorized representatives.
• Educate and support members on available resources and self-advocacy.
• Maintain accurate documentation and ensure policy/regulatory compliance.
• Collaborate with multidisciplinary teams to optimize outcomes.
