Job summary

Community Care, Inc. is seeking a Care Manager Mentor in Madison, WI, to provide on-the-job training and ongoing support to new Care Managers. The role emphasizes a weekday schedule and local travel within Dane County.

The candidate should hold a Bachelor’s or Master’s in Social Work, Psychology, or Human Services and have experience with older adults or individuals with disabilities. Certification and reliable transportation are required.

Qualifications

• Bachelor’s Degree in Social Work, Psychology or Human Services and 18 months experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity.
• Masters’ Degree in Social Work, Psychology or Human Services is acceptable.

Responsibilities

• Provides on-the-job training, orientation and feedback to new Care Managers related to their assigned caseloads and job tasks.
• Serves as an on-going resource to Care Managers and communicates progress to the supervisor.
• Coaches staff to achieve the highest level of evidence-based practice and retains talent by supporting work experiences.
