Job summary

Community Care, Inc. is seeking a Care Manager Mentor in Madison, WI to provide on-the-job training and guidance to new Care Managers, and to support the team in delivering person-centered community care. The role emphasizes collaboration, coaching, and maintaining high standards of practice in a mission-driven organization.

The position runs Monday to Friday, 8:00 AM-4:30 PM, with local travel as part of the responsibilities to support families and individuals across the service area.

Qualifications

• Bachelor’s Degree in Social Work, Psychology or Human Services with 18 months experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity, OR a Master’s Degree in Social Work, Psychology or Human Services.
• Long-term care functional screener certification is required.
• Regular local travel with use of personal vehicle, valid driver’s license and proof of required insurance coverage.

Responsibilities

• Provides on-the-job training, orientation and feedback to new Care Managers related to their assigned caseloads and job tasks.
• Serves as an on-going resource to Care Managers and communicates with the direct supervisor about progress and training needs.
• Coaches staff to achieve evidence-based practice and supports employee development and retention.
• Responsible for job-specific responsibilities listed in the Care Manager job description for Family Care or Family Care Partnership.
