Job summary

Community Care, Inc. is hiring a Case Manager/Care Manager to join our Family Care Program team in Outagamie County, Appleton, WI.

The role focuses on coordinating services for older adults and adults with disabilities, developing person-centered care plans, and ensuring quality care within a mission-driven organization. The position is full-time with a Monday–Friday 8:00 AM–4:30 PM schedule and includes a $5,000 sign-on bonus.

Qualifications

Bachelor’s degree in a human services or closely related area is required.
Minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities; may substitute for a non-human services degree.
Eligibility for long-term care functional screener certification is required.
Regular local travel with valid driver's license and insurance is required.

Responsibilities

Manages an assigned caseload of program members in collaboration with an RN Care Manager.
Completes comprehensive member assessments, member-centered care plans, and functional screens.
Provides ongoing care coordination and quality assurance monitoring of services.
Maintains required documentation in adherence to established standards.
