Community Care is hiring a Case Manager/Care Manager to join our Family Care Program team in Waupaca County. This full-time role emphasizes team collaboration, community-based case management, and delivering person-centered care to seniors and adults with disabilities.

The position offers a weekday schedule (Mon-Fri, 8:00 AM–4:30 PM) and a $5,000 sign-on bonus. Join a mission-driven nonprofit dedicated to independence and quality of life.

Qualifications

• Bachelor’s Degree in a Human Services field required; a minimum of three years’ experience with seniors or adults with disabilities may substitute for a non-human services degree.

This full-time opportunity offers a weekday schedule and a $5,000 SIGN-ON BONUS. Position based in Waupaca county.

Care Manager/Case Manager Job Responsibilities:

• Manages an assigned caseload of program members in collaboration with an RN Care Manager
• Completes comprehensive member assessments, member centered care plans, & functional screens
• Provides ongoing care coordination and quality assurance monitoring of services
• Maintains required documentation in adherence to established standards.

Care Manager/Case Manager Job Requirements:

• Education: Bachelor’s Degree in a Human Services or closely related area required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
• Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.
• Certification: Eligibility for long-term care functional screener certification is required
• Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Care Manager Job Schedule:

• Monday to Friday, 8:00 AM-4:30 PM
