Job summary

Community Care, Inc. is hiring a Case Manager/Care Manager to join our Family Care Program team in Outagamie County, Appleton, WI.

The role focuses on coordinating services for older adults and adults with disabilities, developing person-centered care plans, and ensuring quality care within a mission-driven organization. The position is full-time with a Monday–Friday 8:00 AM–4:30 PM schedule and includes a $5,000 sign-on bonus.

Qualifications

• Bachelor’s degree in a human services or closely related area is required.
• Minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities; may substitute for a non-human services degree.
• Eligibility for long-term care functional screener certification is required.
• Regular local travel with valid driver's license and insurance is required.

Responsibilities

• Completes comprehensive member assessments, member-centered care plans, and functional screens.

This full-time opportunity offers a weekday schedule and a $5,000 SIGN-ON BONUS. Position based in Outagamie county.

Care Manager/Case Manager Job Responsibilities:

• Manages an assigned caseload of program members in collaboration with an RN Care Manager
• Completes comprehensive member assessments, member centered care plans, & functional screens
• Provides ongoing care coordination and quality assurance monitoring of services
• Maintains required documentation in adherence to established standards.

Care Manager/Case Manager Job Requirements:

• Education: Bachelor’s Degree in a Human Services or closely related area required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
• Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.
• Certification: Eligibility for long-term care functional screener certification is required.
• Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Care Manager Job Schedule:

• Monday to Friday, 8:00 AM-4:30 PM
