Community Care, Inc. is hiring a Case Manager/Care Manager to join our Partnership Program team in Racine County. This full-time role supports elderly and disabled adults to live independently in the community through coordinated care planning and services.

The position emphasizes teamwork, client-centered assessments, and adherence to standards. A weekday schedule with a $5,000 sign-on bonus is offered.

Qualifications

• Bachelor's Degree in a Human Services or closely related area required.
• A minimum of three (3) years' experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
• One year of experience with older adults or persons with disabilities in a social service capacity preferred.

Responsibilities

Care Manager/Case Manager Job Requirements

• Education: Bachelor's Degree in a Human Services or closely related area required. A minimum of three (3) years' experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
• Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.
• Certification: Eligibility for long-term care functional screener certification is required
• Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Care Manager Job Schedule

• Monday to Friday, 8:00 AM-4:30 PM
