Job summary

Community Care is seeking a Case Manager/Care Manager to join our Family Care Program in Walworth, WI, with hybrid work options. This full-time role includes a $5,000 sign-on bonus and regular weekday hours (Mon-Fri, 8:00 AM–4:30 PM).

You will collaborate with an RN Care Manager, complete assessments, and develop care plans while coordinating services and ensuring quality standards. A bachelor’s in human services is required, with experience in senior care preferred.

Qualifications

• Bachelor's Degree in a Human Services or related field required.
• Three years’ experience with seniors or adults with disabilities may substitute for a non-human services degree.
• One year experience with older adults or persons with disabilities preferred.
• Eligibility for long-term care functional screener certification required.
• Regular local travel with valid driver’s license and proof of insurance.

Responsibilities

Care Manager/Case Manager Job Responsibilities:

• Manages an assigned caseload of program members in collaboration with an RN Care Manager
• Completes comprehensive member assessments, member centered care plans, & functional screens
• Provides ongoing care coordination and quality assurance monitoring of services
• Maintains required documentation in adherence to established standards.

Care Manager/Case Manager Job Requirements:

• · Education: Bachelor’s Degree in a Human Services or closely related area required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
• · Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.
• · Certification: Eligibility for long-term care functional screener certification is required
• · Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Care Manager Job Schedule:

• Monday to Friday, 8:00 AM-4:30 PM
