Job summary

Trillium Health Resources is seeking a Care Manager – Innovations Waiver to join our Care Management Team. You will coordinate complex care for members with Intellectual/Developmental Disabilities, sometimes with MH/SA needs, ensuring waiver compliance and collaborative planning.

The role emphasizes proactive assessment, multi-disciplinary coordination, and education for members and guardians, with remote options for many positions and a focus on North Carolina counties.

Qualifications

• Must meet QP status and licensure requirements.
• Willingness to travel within NC catchment as required.
• Driver’s license is required.

Responsibilities

• Coordinate complex care for members with IDD and possible MH/SA needs.
• Provide education on services and rights to members and guardians.
• Conduct Discovery and contribute to person-centered planning.
• Inform about Medicaid and state-funded services and options.
• Assist in selecting service providers with objectivity.
