Job summary

Partners Behavioral Health Management seeks an Innovations Care Manager (QP) in Iredell County, NC, a mobile role serving members enrolled in the NC Innovations waiver. You will perform comprehensive assessment, care planning, and coordination of waiver services to support whole-person care in primary, behavioral, and community settings.

The role requires engaging with members and families, updating plans, educating on waivers and budgets, and arranging service authorizations.

Qualifications

• Bachelor’s degree in a health/human services field plus 2+ years with I/DD.
• Or a different degree with 4+ years of I/DD experience.
• RN licensure in NC with 4+ years of I/DD experience.

Responsibilities

• Assess and manage care plans addressing clinical and non-clinical needs.
• Develop and update the ISP (Individual Support Plan).
• Educate members and caregivers on waivers and budget processes.
• Coordinate services to monitor Medicaid and waiver requirements.
• Lead care team meetings and monitor service delivery.
