Job summary

Trillium Health Resources is seeking a Care Manager – RCT to join our team. This role focuses on delivering whole-person care for members with complex physical, mental health, and social needs, coordinating with a multidisciplinary team and external providers.

The Care Manager works remotely within North Central North Carolina, requiring travel to meet with members as needed and availability 8:30 am–5:00 pm.

Qualifications

• Must meet Qualified Professional (QP) status.
• Qualified professionals must meet NC licensure or RN with MH/DD/SA experience.

Responsibilities

• Oversee Tailored Care Management for members.
• Facilitate comprehensive assessments and determine care appropriateness.
• Develop and monitor integrated care plans across physical and mental health needs.
• Lead regular member meetings, in-person or telephonic.
• Consult with providers to share information for care planning.
